Case Management


Every Transition Projects resident works with a Case Manager to develop an individualized plan to obtain housing, employment, and any necessary services, such as alcohol and other drug treatment. Case Managers also connect residents to the resources they need to live productive and stable lives-such as job training, voice mail boxes, bus tickets, and work clothing. Residents meet with their Case Managers weekly to track progress, make case plan adjustments, and receive support.

Besides case managing clients in our buildings, we continue to work with client that we have placed in housing for at least a year after they gain their housing.

Community Voice Mail provides people in need with a personal phone number that looks and sounds like an answering machine. Users most often seek jobs, housing, safety from domestic abuse, and healthcare.

--Clients case managed by Transition Projects may access Community Voice Mail.

--Transition Projects has also served as the host agency for Portland for over 10 years, providing voice mail boxes for other agencies in the community to distribute for their clients.

The Portland Community Voice Mail program is part of the national network of CVM sites, empowering people in crisis and transition by distributing free 24-hour voice mail nationwide—directly linking individuals to jobs, housing, safety and stability.
